The FERPA Release Form is for parents, guardians, or others who wish to speak with OSRR about a student's educational records. Please fill out the form and return to OSRR if applicable.

The Counseling Release form is for licensed counselors, psychologists, psychiatrists, and others who are bound by HIPAA.

Re-Admission Form

This is a form for readmission to the same major for students who may have been suspended from Cal Poly for one year or more.
